I am using BTV with Motorola 2224 STB (the first gen Shaw STB) and Shaw Digital in Vancouver, Canada.

I am using uICE to power an ATI Remote Wonder and HE Remote to power a MyBlaster IR device. The Remote Wonder tells HE Remot (through uICE) to tell the MyBlaster to tell the STB to change channels.

Sounds messy hehe, but it's working great for me.

Generally I have a few complaints about some stability issues in BTV .. but .. given that there is no Tivo or Replay TV in Canada (I believe the only PVR is with Bell ExpressVu) ... I really do wonder how folks get by without a PVR (and I would not consider myself a TV watcher).

It takes some patience and some tinkering to get it all sorted. Wife starting to like it. Daughter can mash the remote keys and somehow bollocks it up... but... it's getting there in terms of being as robust as the Shaw STB / EPG, only much more functional and much better looking.

Note that I spent about 3 months researching, tinkering and testing before putting it into the living room. Depending on your time & living situation (wife and 3 kids in my house) you may be able to get it done overnight .. I just had to be conservative due to the family environment and my goal of having an HTPC be easier to use than components. Still fine tuning & tweaking, but getting there.
