This will require setting up two separate inputs using the same card. Through two visits to the Setup Wizard and Web Admin, you could have this setup and working properly via an ingenious but somewhat messy workaround. For example:
Run through the Setup Wizard and and configure the existing input normally, with the existing lineup, for three-digit padding on the UIRT. Finish the Wizard, and go to the Web Admin (http://localhost:8129). Go to SETTINGS --> CHANNELS. Go through the list of channels for this lineup and check the "Hide?" box corresponding to all four-digit channels. A "Hide All" button exists which will make this a lot easier. Be sure to save the changes.
Run through the Setup WIzard again, create a NEW lineup (the same one as before), configure one of the other inputs on the same tuner card for use with this new copy of the lineup and for four-digit padding on the UIRT (e.g. if you were originally using Coaxial, this time you could use Composite). FInish the Wizard, go back to the CHANNELS section of the Web Admin, and select the second lineup (you'll be able to tell the difference because the one you configured before has all the four-digit channels hidden). On the second copy of the lineup, select "Hide?" for every one, two, and three digit channel. Save the changes and exit.
This will ensure that the three-digit channels that appear in your guide will use the input configured to use three-digit padding. The four-digit channels that appear will use the input configured to use four-digit padding...

I would add that it is easier if you edit your original channel listing and give it a friendly name like "3-digit Channls" or something to that order before you go back and add the second device and channel listing. It makes it much easier to differentiate between the two when you do the work around.
I ended up using the S-Video output from my Dish 5000 for 4-digit channels and the composite out for 3-digit channels. The bottom line is that the solution works and you end up with one seamless program guide that no one, but the geek in the house even knows is using all of this behind the scenes.
